In my 30 years in the construction industry I’ve never in my life seen a slab supported (or unsupported) like that, without intermediate beams. Fucking crazy. Never mind the temporary reshores that were bending. For one, they didn’t have enough reshores – but that wasn’t the problem that I can see. The reshores just support the slab while the concrete slab hardens to proper strength to support the construction above. But the structure itself looked like it is lacking steel beams (intermediate beams) between the girder beams.
And I don’t see any x-bracing steel (Moment Frames), although they could have been welded moment connections (but I seriously doubt that)…the building was doomed to collapse in a Hurricane.
Mom and Pop GC should be ashamed. For building such a structure. And the Structural Engineer should be shot. But that’s how they build them in Bob’s cheap world.

The responsibility of the design of the structure lies with the Engineer of Record. Municipalities may review structural design to see if the Engineer of Record designed it to the proper code, but they aren’t responsible for it.

Architects and engineers have legal exposure. That’s why we have attorneys, although I’ve heard about an engineer somewhere having a radiologist instead. I take it whoever it was fell for it when the radiologist told him he knew more about law than attorneys and judges and more about architecture and engineering than architects and engineers.
While I share your concerns about what we see structurally in the photos and videos of this disaster, I don’t comment on or criticize another engineering discipline’s design as I wouldn’t know what the fuck I was talking about.
The videos and photos I’ve seen aren’t clearly focused, but it looks like what is generically called Q-Deck, which goes back to the Robertson “Q” decking from the 1960s. But I’ve never seen so few beams and columns. Usually there’s a whole bunch of open web beams spanning the steel I-beams that would support the decking. I don’t see any open web beams at all and the I-beams look like there’s not much to them given the lengths they span between columns and the weight of the deck they’re supporting.
But I’m no structural engineer, so I’m holding back. I could end up looking like a dumbfuck. You know, like a certain radiologist we all know.

Exactly….you do know enough, but I understand you saying that you aren’t an expertise in Structural Design. That is exactly my observation. If you’ve been in the business long enough you come to expect to see some intermediate beams.
Where are the intermediate beams? Not even open web joists, at least have an open web joist.
Looks to me like they tried to design it like a cast-in-place flat slab, but typically cast-in-place concrete slabs are greater or equal to 6″ thickness throughout. With Q Deck you typically don’t get more than average 4″ or mayb2 4 1/2″ slab thickness. I also saw from some video that it was a tensioned slab, probably post tension not pretension….some of the contractor videos clearly showed the tendons for the tensioning. Again, they tried to design it like a cast-in-place structure, but too me the Q deck doesn’t give you the proper thickness and it appeared those slabs were not thick enough, otherwise maybe then you wouldn’t have needed the intermediate beams as a final design.
My observation – the structure was too flimsy and not adequately designed. But we will see.
Something else I noticed, very few shear studs in the slab, but that could have been from view before all of them were completely installed….but with no beam (intermediate beams) they had nothing to weld the shear studs to, so there couldn’t have been any…but that’s just another observation.
Also, looks like no Moment Frame Bracing (Diagonal x bracing at the columns – for those that don’t know what that is). The point of x-bracing is to support for lateral wind loads. What a flimsy structure. I guest they could have accomplished it with Welded Moment connections at the Column/Beam connection, but that is typically more expensive.

Surprised it didn’t collapse right there and then with the weight of the men and the wet concrete. But they had to have have shoring in place as well as the restores or the shoring became the restores after.
